T-Shirt Order Forms and Documentation

All forms must be completed in their entirety when submitting your order. Please be accurate on your student count and limit the extra shirts that you order. The forms can be mailed or emailed for a faster response. This year we had several departments that wanted to exchange shirts for smaller sizes, that is not possible. The most responsible way to order is sizing your students first, these are all adult sizes. Most students will fit a small or medium shirt.  Depending on your computer and software, you may not be able to edit the forms but will need to print them and then complete the information. When you order shirts you are agreeing to submit Pre & Post Test Results for all programs.
