2025 - 2026 Planner Order Form

As always we ask that you order only the number of Planners you need, please have an accurate count of the students and include that with your order form. Please use any you may have carried over from last year as we anticipate an updated Planner in 2026.

It is no longer an option to participate in the pre and post-test. Those results must be submitted at the end of your program to continue participation for free planners and shirts.
It is very important to include all numbers of students you are involved with during the school year on the last page. This includes one time lessons, parent meetings, assemblies etc. If you are not certain for the coming year, use last years numbers. No shirt orders will be processed without complete data. Numbers should be close to the certified enrollment as reported to the State of Iowa for your school.
